Thank you for your interest in booking with Hall Veterinary Services, we would love to see your pets! Please fill out this form as completely and accurately as possible.

Due to our busy schedule, our new client appointments are currently being booked for mid-January 2026. We will follow up to your appointment request within 24 hours, during normal business hours.

*Filling out this request form does not automatically book a visit for your pet!*

Checking this box acknowledges that you are aware that Hall Veterinary Services is not booking new client exams until after January 12th, 2026, and are okay with booking after this date.

Pet Information

Payment is due in full at the time that services are performed. We accept cash, Visa, MasterCard, Discover, and CareCredit payments. Beginning 7/1/25, the following fees will apply: A 3% card processing fee will be added to all credit and debit card purchases. A $35 returned check fee will be charged for any check payments that are returned due to insufficient funds or other bank-related issues. Submission of this form acknowledges you have read and accept our financial policies. We neither extend credit, nor bill for services. All open invoices are sent to collections after 45 days unless prior arrangements are made.

 

Clients who cancel their appointment less than 24 hours ahead of their scheduled appointment time, but before the day of the appointment, shall be subject to a “24 hours or less cancellation” fee of $75.00. Appointments that are cancelled the same day (or after-hours the day before) or no-show appointments will be charged a fee of $125.00. This fee will be assessed per appointment scheduled and will need to be paid prior to any additional visits being scheduled.
